Health Office Forms
All medications (either prescribed by your physician or any over-the-counter medications) that need to be taken in school require a note from both the physician and parent/guardian giving permission to the nurse/school to administer the medication. Please contact the office and we will forward the correct form to you.
If your son/daughter needs a physical for sports, please click on "Required NYS School Health Examination Form". This form should be completed by your child's physician and returned to the Health Office prior to the beginning of the season.
