News From the Health Office
Immunization Requirement for School
All 7th graders in New York State are required to be vaccinated against meningococcal disease before starting school. This vaccination is essential for school attendance, and students without it will not be allowed to attend classes. Please ensure you submit proof of vaccination to the Health Office before the start of the school year.
Fall Sports
Fall sport sign-ups will be online Monday, May 12th through Monday, May 19th. The completed Athletic Consent form must be handed in to the Middle School Health Office by Thursday, May 22nd. Students interested in participating in sports must have a current physical on file in the health office. For a physical to be valid for the fall sport season, it must have been completed on or after August 1, 2024. You can email physicals to either jshurmatz@lancasterschools.org
Working Papers
Your child can get their working papers before school ends for the summer. Stop by the health office to pick up an application. Working papers are processed at the Lancaster High School, instructions for Working Papers are available at the following link: Working Papers. Please keep in mind that processing of working papers over the summer months will take 5-10 days due to summer hours and you will need to complete this process at the High School. You do not need to have a job to obtain working papers.
End of the Year Reminders

Students who have medication in the Health Office may pick up their medication anytime before the end of the school year. If the medication is a controlled substance, a parent will need to come to school and pick-up the remaining medication before 10:00 am on Wednesday, June 25, 2025. All medication left in the health office after 6/25/25 will be disposed of.

2025-2026 Medication Forms and Physical Education Restriction Forms will be emailed in June. You could also find these forms on the Lancaster Middle or High School Health Office Website. Please have your child’s physician complete and return the form(s) and return to the school's health office over the summer.

The nurse will be available in the Health Office on August 25, 2025 during the building walk-through from 10:00 a.m. to 2:00 p.m. for parents who wish to drop off medications or forms. Additionally, I will be in the Health Office on August 27 and 28 from 8:00 a.m.-2:00 p.m. for any parents who want to drop off medications or paperwork during those days.
