Physicals:
New York State requires physicals for students in grades: Kindergarten, 1st, 3rd, 5th, 7th, 9th and 11th. Also, any new entrants require a NYS School Health Examination before entering school.
Medications:
The State of New York mandates that the school nurse follow the procedures listed below:
- All medication, including non-prescription drugs, given in school must be prescribed by a licensed medical doctor. Please use the Administration of Medication Authorization Form.
- A written request from the physician must be on file. This request must indicate the dosage and frequency of the prescribed drug. A written request from the parent to administer medication must be on file.
- The parent must assume responsibility for having the medication delivered to the Health Office in a properly labeled original container.
Please do not send any type of medication to school with your child unless you use the proper procedure.
Parents of Seventh Grade Students:
All students entering seventh grade must have the meningococcal vaccine. Please send proof of immunization to the school nurse.
All seventh grade students must have a current NYS School Health Examination completed by a New York State licensed provider on file at school by October 1, 2024.
